Can you tell us about your role with Select Rehabilitation and what a typical day looks like providing therapy services at Meridian at Stone Creek?
As Clinical Lead for our on site Select Rehabilitation clinic, I strive to connect with our clients in helping them reach their goals to optimize their functional independence at their maximum potential. Each day, my team and I greet our clients at their apartments with our equipment on hand or bring them to our on site therapy gym. We do our best to provide sessions at times our clients prefer and make every effort to schedule around facility activities or outings.
What types of therapy services does Select Rehabilitation provide on site at Meridian at Stone Creek?
We offer physical therapy and occupational therapy services up to five days a week. Speech therapy can also be arranged based on therapist availability.
What are some of the most rewarding outcomes you’ve seen with residents here?
One of the most rewarding outcomes we see is helping residents become more confident in living a healthy, independent lifestyle. We understand the importance of independence and want to help our clients preserve it as long as possible. We value guiding residents on a path that improves their strength, mobility, balance, and ability to complete daily tasks on their own.
Watching our clients achieve goals such as walking longer distances, getting dressed independently, or having more energy to spend time with family and participate in activities is incredibly rewarding. It is important to us to maintain strong therapist to client relationships so our clients know we are always here to support them. Giving our clients and their families peace of mind is something we value greatly.
How does being located within the community enhance the therapy experience?
Being on site allows us to become familiar with our clients’ daily routines, assess them in their living spaces, and provide services in our fully equipped therapy gym. We encourage residents to participate in community activities, help them establish routines, and offer flexible scheduling so therapy fits naturally into their day.
In addition to therapy services, we provide a weekly exercise class for Memory Care and Assisted Living residents, quarterly balance screenings using our VSTBalance system, and monthly wellness talks to educate residents and families on topics such as stroke awareness, heart disease, adaptive equipment, and more.
Can family members be involved in a resident’s therapy goals and progress?
Absolutely. Our team is happy to keep families informed of goals, progress, and any barriers so we can work together to set residents up for success. Family support is often a key component in helping clients reach their goals and allows us to share recommendations to ensure they get the most out of their therapy experience.
